Immunization and Tuberculosis Screening Certificate
All new students must submit this form before you can enroll in classes (D.C. law)
Health Insurance Waiver Form
If students have health insurance coverage from another source, they can waive the student health insurance plan. Students must have health insurance while enrolled.
Academic Advisor Change Form
Students who wish to change their academic advisor may do so at any time by submitting this form.
CCT Conference Presentation Grant Application
CCT students may apply for CCT Program funds to assist with expenses incurred when presenting papers at conferences.
Graduate School Conference Grant Applications
Additional conference funds are available from the Graduate School. Please be aware that deadlines for requests are in early September.
Course Permission Form
Use if 1) a course is full 2) a course has registration restrictions. Students must obtain the professor's approval on a course permission form and take this form to the Registrar's office to Register.
Datawatch Agreement
Submit this completed form to CCT's Business Manager to get a key that will give you after-hours access to the Car Barn and CCT Computer Lab.
Free Application for Federal Student Aid (FAFSA)

As a returning student, you won't have to complete these forms from scratch again. You'll be asked to review and update any information that has changed to reapply for federal financial aid.
Declaration of Degree Sequence
Studens declare their intention to follow either a Thesis or Coursework Option curriculum by May 1 of their first year in the program, or the end of the second semester in the program, (whichever applies).

Transfer Course Request Form
Students must submit this form to the Academic Program Manager for internal CCT approval of transfer credits from institutions outside of Georgetown University. This form isn't necessary for classes taken through the DC Consortium.
CCT Video Equipment Check-out Form
Bring this form to the CCT Lab in order to check-out the digital video camera and related equipment for student projects.
Business Cards
Students are able to purchase their own Georgetown business cards through the university.
Institutional Review Board
Institutional Review Board (IRB) Informed Consent Samples for any kind of course or thesis research involving human subjects (surveys, interviews, etc.)
Thesis Proposal Form
Students pursuing the thesis option must obtain approval of a CCT thesis advisor on this form to pursue the thesis. The Thesis Proposal Form is the thesis advisor's approval of working with you on the thesis. A reader's signature is not required when you submit this form to the Academic Program Manager (Nov. 1 for spring semester thesis writers and April 1 for fall semester thesis writers).

The Thesis Timeline has a summary of Graduate School required and CCT-recommended thesis deadlines, as well as links to required forms and thesis resources. Faculty are encouraged to use this form as a framework for a Thesis Colloquium syllabus, adding their requirements and expectations for the course to the Graduate School deadlines already listed.

Graduation Forms
In order to graduate you will need to submit your forms before the first day of the month of your graduation. May graduates must submit forms before the end of the add/drop period in January.
Book Orders via University Bookstore
Course texts are ordered through the Georgetown University Bookstore in the Leavey Center on the main campus. The bookstore has recently launched a web-based system for Faculty Book Orders, which you are encouraged to use. You will be redirected to the Georgetown-affiliated efollett.com site.

Incomplete Grade Form
If you give an Incomplete (I) grade to a CCT student, please fill out this form, and return it to the Academic Program Manager. Alternately you can email her with the deadline by which the student must complete the coursework.
NetID Password Change Form
Submit a password change form by fax, accompanied by a copy of a photo ID, and UIS will call you with your new password.
Course Reading Reserve Forms
Book Reserves / Electronic Reserves:
Place articles or books on reserve at the library, so your stduents can access them. Electronic reserves are a preferred option for articles, as students can access them online from anywhere with Internet access.
